PARENTS 'IN SHOCK' AND ASKED DERRIFORD TO KEEP HIM IN
A TEENAGER in Cornwall died just hours after he was discharged from hospital, an inquest has heard.
Jacob Chapman was 18 when he died in Liskeard on May 22, 2024.
The first day of a three-day inquest into his death heard the teen had struggled with his mental health for a number of years and, in the days leading up to his death, had expressed suicidal ideations and attempted suicide twice.
His parents had desperately sought to help him, working with emergency services to try to keep their son safe.
