SHRC probes SMS Hosp after fatal blood transfusion error

The Rajasthan State Human Rights Commission (SHRC) has taken suo motu cognisance of a medical negligence case at SMS Hospital, where a 23-year-old pregnant woman from Niwai, Tonk, died after being transfused with wrong blood group.
The woman, admitted on May 12 with TB-related complications, was 5 months pregnant and on ventilator support when she was mistakenly given A+ blood instead of her actual B+ type on May 19. The transfusion triggered a severe reaction and despite immediate efforts, she could not be revived.
This incident marks the third such case since Feb 2024 at SMS Hospital, raising alarming concerns about systemic lapses. A key failure included the absence of blood group information on the requisition slip and lack of proper sample verification before issuing blood.
