MAMMOTH Matthew Perry’s hankering for junk food and cigarettes is sabotaging his self-proclaimed effort to get healthy, say sources.

The Friends alum, 54, was looking particularly slovenly while shopping at a Tinseltown Nike store with an assistant late last month. He arrived wearing shabby blue sweatpants, a long-sleeved gray shirt and sneakers — and left in a new polo shirt and shorts.

But the spiffier duds didn’t seem to make much of a difference.
