New 'care co-ordination hub' is designed to help NHS patients stay at home

AN NHS hub in South Tyneside is fielding 32,000 calls a month as part of a drive to look after people in their own homes rather than in hospital.
The "care coordination hub" serves South Tyneside and Sunderland and brings together a number of community-based NHS teams.
Patients and NHS figures - such as GPs or ambulance staff - have a single number to call and then they can speak to anyone from a district nursing team to a therapist or even a GP working as part of the "hospital at home" programme, previously known as a "virtual ward". The idea is to help people get the support they need without having to call 999 or head for accident and emergency.
