Care plans drawn up to help people in their homes

THE BETTER CARE FUND PLAN HAS OVER £30M IN FUNDING

CARE plans have been drawn up to help people in their homes, use technology and support unpaid carers with over £30m of funding.

Stockton Council has sent its Better Care Fund plan for the next year to NHS England. It is part of government reforms to shifting "from sickness to prevention and from hospital to home", stop people going into hospital unnecessarily, help them recover in their homes and have fewer people going into residential homes, using money from grants and NHS contributions to adult social care totalling £31.5m.

Yvonne Cheung, the council's transformation manager, said: "We've submitted the plans for 2025-6 to NHS England. We are going to set up a governance arrangement, a local oversight group, and they will include the representatives from the key partners.
