Three packed passenger planes narrowly missed smashing into a ladder on a busy runway after it fell off a maintenance truck.
Investigators said it could easily have caused a serious accident if hit by any of the aircraft, which were travelling at about 140mph as they landed.
Two bungling electricians failed to spot that the 7ft-long ladder had gone missing as they checked runway lights at Birmingham Airport.
