Young carer Grace takes a ramble for BBC Children in Need

A Perthshire teenager will appear on TV this Sunday exploring the countryside as part of a special programme for BBC Children in Need.

Chat Countryfile presenter Sean Fletcher joins Grace for the walk

Sixteen-year-old young carer Grace, from Perthshire, joins Countryfile presenter Sean Fletcher for the walk.

Grace helps to care for her mum, who continues to face medical challenges following a head injury, as well as her twin sister, who has additional needs.

Her days are often filled with responsibilities - walking her sister to school, preparing meals, cleaning, shopping - doing whatever she can to keep things running smoothly at home.

Grace has been supported by charity PKAVS (Perth and Kinross Association of Voluntary Service), which helps people in the community, including young carers, and is part-funded by BBC Children in Need.

PKAVS has provided Grace with opportunities to take part in fun activities, access counselling and school tutoring, and connect with other young carers who understand what she's going through.
