HALF of the firms running homes to prepare vulnerable youngsters for life outside the care system are failing, an Ofsted report reveals.

It comes after a teenager died in supported accommodation where only six out of 78 staff had received self-harm training.

Ofsted found the company involved - Forever Care (North East) - guilty of “significant breaches”, with children “at risk of significant harm”.

It also suspended the registration of the firm, which made a profit of £1.5million last year.
