HSE apologises for 'failings which led to tragic Niamh's death'

THE HSE has apologised unreservedly to the family of a teenager who sadly died at University Hospital Limerick, following "failings".

An inquest into the tragic death of Limerick teen Niamh McNally, 16, who died in the emergency room of University Hospital Limerick in January 2024 after being rushed to hospital with serious breathing difficulties, opened at Limerick Coroner's Court yesterday.

The inquest, attended by Niamh's mother Carolyn, her grandfather Clem, uncle Pete and her best friend Naomi, began with an apology read on behalf of the HSE Midwest, apologising unreservedly for the failings which led to Niamh's death.

"The circumstances surrounding Niamh's passing are a source of immense sorrow for us, and we sincerely regret the opportunities that were missed to intervene," the apology read.
