BEACH Boys legend Brian Wilson is a dementia-ravaged shut-in who needs 24/7 care, according to a heartbreaking dossier from his doctor detailing the rocker’s sad last days!

Wilson’s condition is so dire, a Los Angeles Superior Court recently approved a petition from his inner circle to place the ailing 81-year-old musician under a conservatorship to manage his affairs!

As part of the proceedings, Dr. Stephen S. Marmar presented fraught findings, revealing the onetime musical genius — who co-founded the band and was behind iconic hits including Good Vibrations, God Only Knows, Surfin’ U.S.A. and Fun, Fun, Fun — has lost his mind due to “a major neurocognitive disorder.”

Marmar’s five-page declaration, obtained exclusively by GLOBE, lays bare Wilson’s tragic existence, stating the addled artist cannot care for himself and is “incapable of self-administering medications.”
