LUPUS IN FOCUS: NAVIGATING DELAYED DIAGNOSIS AND EMERGING TREATMENTS

Systemic lupus erythematosus (SLE), commonly referred to simply as lupus, is a chronic, multi-system autoimmune disorder that remains both a fascinating medical challenge and a pressing public health concern.

It is a condition where the body's immune system designed to protect against infections turns against its own cells and tissues. This misguided response can affect virtually any organ, from the skin and joints to the kidneys, heart, lungs, and brain.

The name lupus dates back to the 13th century, derived from the Latin word for "wolf," referring to the facial rash thought to resemble a wolf's bite, and erythematosus meaning "redness." While the historical name evokes imagery, the modern medical understanding is rooted in immunology and molecular science. Today, lupus is recognized not as an immune deficiency but as an abnormal immune response triggered by a combination of genetic predisposition, environmental exposures, and immune dysregulation.

Epidemiology and Patient Profile

Lupus affects people of all ages, though it is most common in women of childbearing age, between 15 and 40 years. The female-to-male ratio is striking approximately 9:1 likely due to hormonal influences, particularly estrogen. While global prevalence varies, estimates suggest that 20-150 per 100,000 people are affected, with higher rates in certain ethnic groups, including Asians, Africans, and Hispanics.

In India, lupus is underdiagnosed, partly because of limited epidemiological studies, fragmented healthcare access, and low awareness among both the public and nonspecialist clinicians. In many cases, patients endure years of vague symptoms before a definitive diagnosis is made.
