Jack with his special red bench in Gedling Country Park
Jack Taylor, 19, from Nottingham, was given a less-than-10% chance of survival and it is hoped the bench will help to raise awareness of the importance of research into cardiovascular disease.
The bench with Jack's name on it has been unveiled at Gedling Country Park - where Jack and his family have enjoyed family days out.
He said: 'I'm delighted to be part of this campaign to help spread awareness of cardiovascular disease.
