THE deaths of five people on a dive to the Titanic were the result of “inadequate engineering”, a US investigation has found.

The OceanGate Titan submersible imploded near the wreck of the ship in 2023.

Now US federal investigators have concluded poor engineering and a failure to test the strength of the vessel under pressure were to blame for the tragedy.
