Probes reveal the tragic human cost of failures

POLICE opened a probe into the death of Jake Llewellyn at Forth Valley Royal Hospital last year.

The Glasgow writer, who had bipolar disorder, was admitted eight days before his death. Medics were forced to restrain him and gave him sedatives but he later died. A whistleblower claimed last year he had been left unsupervised and there had been other serious incidents on mental health wards that led to fatalities.
