CHARLIE Millers was being checked on every five minutes before his death, Greater Manchester Police told his inquest.

But that major conclusion is not based on documentary evidence, the force admitted.

GMP said they were ‘unaware’ of vital documentation meant to have been completed by mental health unit staff, with some of the results of a reinvestigation into the death of the teenager based on interviews with staff.

The inquest into Charlie’s death was delayed by more than a year after police requested the hearing to be postponed so GMP could reopen its investigation after it emerged hospital records may have been edited after his death.
