A GRANDMOTHER who died after suffering a stroke was discharged from hospital without her medication, an inquest heard.

Mary Powell, 91, was admitted to hospital with pneumonia before she was transferred to a community hospital for rehabilitation.
An inquest heard Mrs Powell’s family brought her home in an unplanned discharge after being unhappy with the conditions on the ward.
But Mrs Powell had been taking new blood thinners during her stay - and her family were given no supplies when she left on a Saturday.
Daughter Elaine Muir said she continued with her mum’s usual medication at home and had no idea she had to stop giving her aspirin.
The inquest heard the family also did not realise they needed to request a new prescription for blood thinner Edoxaban to reduce Mrs Powell's risk of stroke.
