NHS Forth Valley was forced to say sorry after the patient&#39;s child complained to the Scottish Public Services Ombudsman (SPSO).The patient, who had a cognitive impairment, was in Falkirk Community Hospital and got access to detergent in the staffkitchen. After falling ill, they were transferred to Forth Valley Royal Hospital but died a week later. A Significant Adverse Event Review (SAER) was launched but was unable to establish if detergent was ingested and contributed to the death.The relative complained to the SPSO about inaccuracies in the review and clinical records. An inquiry found the transfer was not formally documented, there was a delay in staff completing an incident report and the SAER and in responding to the complaint.A spokesperson for NHS Forth Valley said: &ldquo;We have apologised to the family and NHS Forth Valley has taken action to address all the recommendations in the report&quot;.
