WHAT IT ISPhotodynamic therapy (PDT) is an in-office procedure to treat stubborn acne, especially the cystic and inflammatory types. The practitioner first applies a form of aminolevulinic acid&mdash;usually Levulan Kerastick or Ameluz&mdash; that&rsquo;s absorbed by acne-causing oil glands. The patient is then exposed to bright light to activate the medication. Both work to shrink oil glands, leading to fewer breakouts. Traditionally, patients have been treated with blue light, but according to research by New York dermatologist Macrene Alexiades, MD, PhD, published in the Journal of Drugs in Dermatology, red light is better at targeting acne. &ldquo;After Accutane, it&rsquo;s actually the strongest treatment that exists right now,&rdquo; says Alexiades, who notes that PDT can be used on teens as well as adults, and at any time during the breakout cycle. Aside from Accutane&rsquo;s known potential to cause serious birth defects (requiring female users to use contraception), it can also leave deep scars once the condition has cleared. But according to Alexiades, PDT has the potential to banish scarring by activating a protein that stimulates the growth of new blood vessels.COST Typically between $800 and $1,500 for one session; two to three treatments are often required to see long-term results.WHAT I EXPECTED I began my battle with breakouts in middle school. I went on Accutane, which improved my acne but didn&rsquo;t cure it. According to Alexiades, the ideal PDT patient has &ldquo;inflammatory acne&mdash;red, pustular cysts&mdash;and red scars,&rdquo; so I fit the bill perfectly. I hoped PDT would help get rid of the stubborn cystic spots ringing my face, like along my jawline and outer cheeks. I also wanted it to smooth some of my scarring, but I was worried about pain and redness.WHAT IT&#39;S ACTUALLY LIKE The Levulan, a milky lotion, is swabbed all over my face, and then I wait an hour in Alexiades&rsquo;s office while the medication binds to my oil glands. (It dries clear, so during that time, you can go out and grab lunch, walk around, or just hang. I choose to watch Parks and Recreation on my phone.) Then I recline in a cushioned chair, don a pair of protective goggles, and am exposed to red light&mdash;shining on me from forehead to jaw&mdash;for 16 minutes. There is no pain. It feels more like a warm sunbath. To prevent any redness, I finish with ice packs for a few minutes. I also leave with instructions to avoid using makeup or any skin products for at least two days because my skin is especially sensitive. Although my face is nowhere near as inflamed as I expected it to be&mdash;it resembles a slight sunburn&mdash;I still shroud it with a baseball cap and blanket scarf for my Lyft home. I use this experience as a solid excuse to spend the weekend inside, in low light, ordering Mexican food and watching Netflix.THE RESULTS On day two, I wake up with a few rough spots and whiteheads, followed by peeling and a week or so of breakouts as my oil glands dissolve. After about seven days, the acne fades. By the two-week mark, I count zero blemishes&mdash;a far cry from the double-digit numbers I&rsquo;d previously dealt with on a daily basis. I&rsquo;ve still got scars, but my cystic acne seems like it&rsquo;s cleared up completely&mdash;something I couldn&rsquo;t say post-Accutane. New zits are much fewer and far between now, and I&rsquo;m already saving money on concealer.
