'We're all human and make mistakes' dad tells medics after error that led to baby's death

TUBE HAD BEEN INSERTED INTO WRONG VEIN OF 'CRITICALLY-UNWELL' PREMATURE INFANT
A FATHER has told medics at a Nottingham hospital to “forgive yourselves” after a mistake led to the death of his premature baby.
Two-month-old Erioluwa Ajala died at the Queen’s Medical Centre last June after what was supposed to be a routine procedure to insert a tube used to deliver medication and nutrients.
However, the tube was put in the wrong place and when medics started to use it without realising, it resulted in a serious spinal injury.
The error caused irreversible damage to Erioluwa, who died around two months later.
An inquest, which concluded on Wednesday, found medics missed two chances to recognise the mistake through X-rays.
Proceedings were told the premature baby could have survived had the error been identified, but the baby’s dad, Lawrence Ajala, urged the staff involved in the care to forgive themselves.
Emotionally addressing some of the workers at Nottingham Coroner’s Court, he said: “I love you all. I don’t know what to say but I love you all.
