The Parliamentary and Health Service Ombudsman (PHSO) said the Department for Work and Pensions (DWP) failed to act quickly enough once it knew a significant proportion of women were unaware.
The 1995 Pensions Act equalized the pension age for men and women. It is 66 and will rise to 67 between 2026 and 2028.
The ombudsman said it had received a lot of complaints. Many women said they suffered distress and financial loss.
